Youth Football Liability Waiver
The undersigned is a parent or legal guardian of the player registered on this form. I fully understand that football is a contact sport and that serious injury to the child may occur through participation in this sport. In consideration for allowing my child to participate in the Sioux Valley Football Association youth football program I agree to hold the Sioux Valley Football Association, officers, coaches, Sioux Valley Public Schools, Volga Christian Schools and any person affiliated or working with or for either entity, as a volunteer or paid employee, harmless against any claim on behalf of the child which results from the child’s participation in the SVFBA youth football program whether such participation involves football, transportation or any other activity in any way connected with the child’s participation.

I further understand that this is a release of liability and, with my signature, I am attesting to my authority to sign on behalf of the child and to personally indemnify any of the released parties in the event a claim is made by or on behalf of the child for any claim released by this form. I understand that I will be personally responsible to hold the released parties harmless and indemnify them by paying them for any damages they are required to pay or cost, including, attorney’s fees, they incur in defending against claims made by or on behalf of the child if those claims arise in any way from the child’s participation in the football program.

Youth Football Code of Ethics
1. A parent/guardian will not criticize the coaches, players, opposing teams, fans or officials, whether publicly or privately.

2. A parent/guardian will not interfere with the coaching methods of the coaching staff.

3. A parent/guardian will refrain from using abusive and/or profane language

4. A parent/guardian will abide by school district policy regarding the possession and/or use of illegal drugs and alcohol on school property.

5. A parent/guardian will instill good sportsmanship in their player.

6. A parent/guardian will not strike any coach, official, spectator, or participant in the Sioux Valley Football Association youth football program.

7. Any comments or concerns by a parent/guardian should be directed to the Head Coach or to the officers of the organization as soon as possible.

8. Parent/guardian will be responsible for their child’s behavior at all football activities.

Photo Release

I, (Parent/Guardian Name), hereby authorize Sioux Valley Football Association to photograph me and/or my child/property and authorize Sioux Valley Football Association, its legal representatives, successors and assigns the absolute right and unrestricted permission to copyright, publish and/or use such photographs or statement, in whole or part, or composite, or distorted character of form (including use and publish with my name or no name) made through its studios or elsewhere, for art, advertising, trade or any other lawful purpose.

I hereby waive any right that I may have to inspect and approve the finished product or the advertising copy that may be used in connection therewith, or the use to which it may be applied.

I hereby release, discharge and agree to hold harmless Sioux Valley Football Association, its nominees or others for whom Sioux Valley Football Association is acting, from any liability by virtue of any use whatsoever, whether intentional or otherwise, that may occur or be produced in the taking of said picture, or in any processing tending toward the completion of the finished product.

Social Media Guidelines

All SVFBA Members shall abide by the following guidelines when using social media:  

1.  Be positive and respectful, and always take the high road.  When disagreeing with others' opinions, remain appropriate and polite.  If you find yourself in a situation online that is becoming antagonistic, ask the SVFBA board or your coaches for advice on how to disengage from the dialogue in a polite and respectful manner that reflects well on SVFBA.

2.  Do no post content that would harm the SVFBA, any member, parent, sport coordinator, sport committee member, coach, any other sports program, or damage the SVFBA's reputation in any way.  Remember that even while you are on your own personal time, you are a representative of the SVFBA and people may interpret your online postings or social interactions as though they were official SVFBA statements.  

3.  Use good judgement when posting comments on any official SVFBA sites.  Bear in mind that your comments can create liability for the SVFBA. If you are unsure whether a comment is appropriate to post, either do not post it or obtain prior approval from the Board or coaches.
